Anxiety disorders — generalized anxiety disorder, panic disorder, social anxiety, and specific phobias — co-occur with substance use disorder at rates roughly two to three times higher than in the general population.
Substance use that started as a way to manage symptoms eventually creates its own problem. These are the patterns we see most often in patients with co-occurring anxiety and addiction:
Integrated treatment addresses both the anxiety driving use and the substance use that is now amplifying anxiety. The clinical sequence typically involves medical stabilization (especially critical for benzodiazepine taper), structured behavioral therapy targeting both conditions, and — where indicated — non-addictive psychiatric medications (SSRIs, SNRIs, buspirone) that treat anxiety without dependence risk. Benzodiazepine prescribing for patients with active SUD is generally avoided.

Different condition pairs respond to different therapies. The modalities below have the strongest evidence base for co-occurring anxiety and addiction.
