DBT's structure is four interlocking skills modules. They build on each other and they're meant to be practiced, not just learned.

Mindfulness

Building moment-to-moment awareness — the foundation skill that the other three modules depend on. Mindfulness in DBT is practical and skills-based, not abstract.

Distress tolerance

Surviving high-emotion moments without making them worse — getting through cravings, panic, anger, and shame without acting on them. The skills that prevent a hard hour from becoming a relapse.

Emotion regulation

Identifying, labeling, and changing the intensity of emotions before they hijack behavior. Includes opposite action, building positive emotion, and reducing vulnerability.

Interpersonal effectiveness

Asking for what's needed, saying no, and maintaining relationships under stress — the relational skills that get destroyed by addiction and have to be rebuilt.

Common questions about DBT.

How is DBT different from CBT?

CBT focuses on the thought layer between trigger and behavior. DBT adds an explicit focus on emotion regulation, distress tolerance, and interpersonal skills — designed originally for patients whose emotional intensity made standard CBT less effective. Today DBT is also one of the strongest treatments for SUD when paired with intense emotion dysregulation or borderline patterns.

Do I need a borderline diagnosis to benefit from DBT?

No. DBT was originally developed for borderline personality disorder, but its skills modules are now used widely for SUD, eating disorders, PTSD, and intense emotion dysregulation regardless of formal diagnosis. The skills work because emotional intensity is human, not pathological.

Does Kentucky Medicaid cover DBT?

Kentucky Medicaid plans typically cover DBT as part of integrated behavioral health care for SUD and co-occurring conditions, across residential, PHP, IOP, and outpatient levels of care.

Is DBT individual or group?

Standard DBT uses both — weekly individual sessions plus a weekly skills group. The group teaches the four modules; the individual session applies them to the patient's specific patterns. Our programming uses both formats together.
